She has 25 years of experience in different nursing areas, including home health, acute care, labor and delivery, skilled nursing, and nurse delegation. Three Rivers Nursing Services provides all the services by proper planning, assessing, implementing, and evaluating your requirements to provide customized care. We collaborate with other healthcare entities to ensure compassionate and personalized care plans that address both medical and emotional well-being.

What tasks are not eligible for Nurse Delegation?

  • Sterile procedures or processes
  • Injectable medications (except insulin and non-insulin injections for Diabetes)
  • Central line or IV maintenance
  • Acts that require nursing judgment

Billing

Private Pay or State Pay

Private pay clients pay at their own expense for all services they received in the adult family home, supported living and assisted living facility.

State Pay

Private pay clients pay at their own expense for all services they received in the adult family home, supported living and assisted living facility.

State pay clients receive services from Washington State under Medicaid or DSHS. Each client has an assigned Social Worker or Case Manager who in turn contacts the Registered Nurse Delegatory and starts payment through the Provider One services. Please contact the assigned SW (Social Worker) or CM (Case Manager) and inform them of the nurse delegator’s name and phone number or send us a message.

Understanding the basics of Nursing Delegation

What is nurse delegation?

Nurse Delegation, under Washington State law, allows credentialed Nursing Assistants and Home Care Aides working in a Community-Based Residential Care setting to perform certain tasks—such as administration of prescription medications or blood glucose testing – normally performed only by licensed nurses. A Registered Nurse must teach and supervise the Nursing Assistant or Home Care Aide, as well as prove nursing assessments of the client’s condition.

Three Rivers Nursing Services provides these services in compliance with WAC 246-840-910 through WAS 246-840-970. Our services include initial assessments, care planning, competency evaluations for nursing assistants, obtaining client consent, ongoing guidance, and supervisory visits.

When are nurse Delegation services necessary?

  • The client is unable to perform the last step/get the medication where it needs to go.
  • The client is unaware that he/she is taking medication
  • The client has dementia or cognitive impairment and does not know how/when to ask for the appropriate “as-needed” medications
  • The client requires the caregiver to puncture the skin for blood sugar monitoring or insulin administration
